The Communities for Health model goes beyond traditional employee wellness programs by combining personalized health screenings, lifestyle coaching, and access to YMCA evidence-based programs. Instead of relying solely on workplace resources, this initiative supports participants at work, at home, and within their community.
Employees receive customized text messages, a diagnostic screening, and ongoing guidance from a dedicated lifestyle manager who helps them follow a tailored wellness plan.
How Employees Participate
1. Workplace Enrollment
Wellness champions in each workplace encourage employees to enroll through a simple online platform. Participants immediately begin receiving personalized reminders and access to wellness tools.
2. Personalized Health Coaching
After completing an on-site health screening, employees are matched with a lifestyle manager who recommends the appropriate wellness pathway and provides ongoing support.
3. Access to YMCA Programs
Participants gain access to evidence-based chronic disease prevention programs, group fitness classes, and healthy-living workshops at their nearest YMCA.
