At the Y, we’re committed to serving our members and program participants.

We are aware that membership, program fees, and other Y services may be tied to employer benefits or other personal business and want to ensure that if you need records from us, that they are accurate.

Your privacy is important to us so we ask for you to complete this form each time you require records. The Y will only release information to the member or program participant to whom the information is related. Please note that even though we understand you may have recurring needs for these items, we cannot do an automatic and/or recurring records release. You must complete this form each time you have a records request.

Please allow 7–10 business days for processing. All records will be emailed to the address listed on the form.

The YMCA of Central Florida has a mandatory check-in process for all members as part of our facility access control system. It is the responsibility of the member to ensure their information is captured at each visit. The data attached does not constitute a metric for member usage or “exercise” as it only reflects access into a Y facility. Furthermore, this data is meant to be internal, and, as such, the accuracy of this data for any purposes outside of the Y may be limited due to member practices, operational, or technological disruptions that can impact data capture and reporting.
